Online Nomination Form

(PHOENIX, Ariz., Nov. 20, 2003)—Do you know a Phoenix College teacher who has gone above and beyond in their profession to help students, pursue higher education or serve as a Phoenix College representative to the greater community? If so, you can thank this special teacher by nominating him/her to receive the Distinguished Teaching Award: Recognizing teaching excellence at Phoenix College.

Exclusive to Phoenix College, this special award allows for recipients (up to two annually) to attend an all expense paid trip to a professional conference or seminar—valued up to $1,500 and share their methods and techniques with other faculty. Also, they are awarded a $250 scholarship given in their name to a deserving student of their choice and are honored with a plaque.

In order to be eligible for the Distinguished Teaching Award, nominees must:

--Be residential (full-time) faculty.
--Have taught at Phoenix College for at least three years.
--Demonstrate a sustained contribution to the students and the college.

If you have a nominee in mind, write a short essay (approximately 250-words) explaining what you believe distinguishes this teacher from others on campus. Be sure to provide specific examples that illustrate this person’s teaching excellence. Also, include your name (if you wish to remain anonymous, please specify), your signature, your relationship to nominee (e.g., student, colleague, etc.), and the date.
