As a new student to Phoenix College, or if you have not taken courses in the past four (4) semesters, you must complete or update your student information online. In addition, you may fill out a Student Information Form [PDF] and turn it in to Admissions and Records. You will also need the Degree/Certificate codes [PDF] to assist in completeing the Student Information Form.
