The student must:
1. Attend an orientation.
2. Phoenix College Admissions
3. Complete Prerequisite Classes with a grade of "C" of better
- Complete a Phoenix College Student Information form (application) and submit it to Admissions and Records before taking ASSET test.
- Complete the ASSET placement test in the Testing Center. College transcripts indicating passing grades in English, Reading and Math courses may substitute for the ASSET test.
4. Upon completion of orientation and prerequisites with a 'C' or better, a
- WED165 - Overview of Massage Therapy
- WED151 - Overview of Alternative Medicine
- BIO160 - Intro. Human Anatomy/Physiology
- HCC130 - Fundamentals in Health Care Delivery
- HCC145AA - Medical Terminology for Health Care
- WED215 - Selfcare for Massage Therapists
5. Successfully complete interview for selection into Program:
- October 8, 2003 - For entrance into Spring Semester
- Schedule Interview appointment at (602) 285-7649
6. If selected
for the program, submit the following to Dianne Geddis G103, South
Gym by August 15, 2003(Fall Core Entrance) or January 12, 2004(Spring Core
Entrance)
- Completed Health Declaration Form including:
- MMR & Varicella immunization
- Recent TB skin test
- Hepatitis B immunization or waiver
- Proof of Health Care Provider CPR certification
