Registration Information

Step 1: Make application to the college if you have not previously taken courses at Phoenix College.

Step 2: If you do not have previous college experience, you will need to be assessed at any testing center in the district for Math, English and reading skills.

Step 3: Schedule MMR Immunization (Measles, Mumps, Rubella, Varicella) and TB skin test or produce valid records.

Obtain CPR certification (health care or professional rescuer level)

Step 4: Make an appointment with the Allied Health Advisor (Debi Moser (602) 285-7427) or Program Director (Julie Stiak (602) 285-7735) to register and bring the following documents:

  • Copy of High school diploma or GED
  • Assessment scores or copies of college transcripts
  • Immunization records (MMR)
  • TB skin test results within the year
  • Hepatitis B Vaccination or Waiver
  • CPR card (health care or professional rescuer level)

Step 5: Pay fees immediately or before fee deadline when you will be purged for lack of payment and have to start over.
