Step 1: If you have not previously taken a course at Phoenix College, submit an application.

Step 2: If you do not have previous college experience, you need to be assessed at any testing center in the district for Math, English and Reading skills.

Step 3: Schedule MMR Immunization (Measles, Mumps, Reubella, Varicella) and TB Skin Test or produce valid records. Obtain health care provider level CPR certification.

Step 4: Make an appoint ment with the Allied Health advisor (Debi Moser 602-285-7427) or program director (Julie Stiak 602-285-7735) to register and bring the following documents:

• Copy of HS Diploma or GED
• Assessment scores or copies of college transcripts
• Immunization Records (MMR)
• TB skin test results within the year
• Hepatitis B vaccination or waiver
• CPR card (health care provider level)

Step 5: Pay fees before fee deadline or you will be purged for lack of payment and have to start over.
