
The application period for the Fall 2009 semester has ended.
The application period for the Spring 2010 semester has begun. Applications are due by 4pm November 20, 2009 to a Phoenix College Adviser.
Step 1:
If you have not previously taken a course at Phoenix College, complete the Student Information Form and submit it to the Admissions and Records Office.
Step 2:
If you do not have previous college experience, take the ASSET or COMPASS test to assess your math, English and reading skills. These tests can be taken at any Maricopa Community College testing center, including the Phoenix College Testing Center.
Step 3:
Schedule immunizations (click for locations and cost) or produce valid records and complete the Program Immunizations and Health Declaration Form. Obtain CPR certification (healthcare provider or professional rescuer level). Submit an application for a fingerprint clearance card (obtain application from healthcare advisor) approximate cost, $65.
Step 4:
Meet with a Healthcare Adviser to begin the application process. Bring the following with you when you see the adviser:
- Completed Phlebotomy Program Application (click here to download an application)
- Copy of HS Diploma, GED, assessment scores, OR college transcripts
- Immunization Records and Health and Safety Documentation Form and supporting documentation
- CPR card (healthcare provider or professional rescuer level)
- Fingerprint Clearance Card
Students admitted into the program will be required to complete a drug screening test (approximate cost, $35) prior to clinical externship.
Step 5:
Pay tuition and fees before the deadline to avoid being dropped. Students dropped for non-payment may lose their spot in the program. Financial aid may be available for qualified students. Get an application packet from the Financial Aid Office or online. The approval process takes six to eight weeks, so apply early!
