As a new student to Phoenix College, or if you have not taken courses in the past two (2) semesters, you must complete the Student Information Form [PDF] before registering for classes at Phoenix College. You will also need the Degree/Certificate codes [PDF] to assist in completeing the Student Information Form.

After completing the Student Information Form, return this form to the Admissions and Records Office.
